Before You Begin
To open and complete a fillable PDF form, we recommend using Adobe Acrobat Reader. If you do not already have it installed, you can download it free from Adobe’s official website.

- Save the PDF file to your computer before filling it out. We recommend saving it somewhere easy to find, such as your Desktop or Documents folder.
- We recommend opening the saved PDF directly in Adobe Acrobat Reader rather than filling it out inside a web browser, as some browsers may have issues with fillable PDF forms.

Downloading Your Form

Tina’s Tiny Clubhouse will send your PDF forms to you by email.

Download the PDF attachment from your email.

Choose where you want to save the PDF on your computer, such as your Desktop or Documents folder.

Once the file is saved, open it using Adobe Acrobat Reader.
Section 3
Opening the Saved Form
Open the saved PDF using Adobe Acrobat Reader. If necessary, right-click the saved file and select “Open with Adobe Acrobat Reader.”

Filling Out the Form
You can type information directly into the fillable fields or copy and paste information from another document. Please keep the following formatting guidelines in mind:
- The font is preselected and cannot be changed.
- Use regular text with upper- and lowercase letters.
- Avoid underlined, bold, italic, or script formatting.
- Avoid formulas or special formatting.
- Avoid curved or slanted apostrophes.
- Avoid double quotation marks.
- Avoid long dashes.

Saving Your Completed Form
After completing the form, save the final version of the PDF. When saving:

Select “Save As” from the File menu.

Name the file using your child’s full name.

Save it somewhere easy to locate.
You can reopen the saved file later if you need to make changes or add additional information.
Section 6
Submitting Your Form
Once the form is completely filled out:

Print the completed PDF.

Sign the printed document where required.
- 3
Bring the signed form to Tina’s Tiny Clubhouse.
